Работа посвящена оценке результатов собственных исследований санитарно-микробиологического мониторинга объектов внешней среды в разнопрофильных лечебно-профилактических учреждениях стационарного типа и анализа отечественных и зарубежных данных в целях обоснования перечня приоритетных контролируемых санитарно-микробиологических показателей воздуха и поверхностей для обеспечения безопасности внутрибольничной среды медицинских организаций стационарного типа (МОСТ) вне зависимости от их функционального назначения.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проводились исследования помещений различного назначения в лечебно-профилактических учреждениях стационарного типа в течение двух лет. Исследования включали определение микробной обсеменённости воздушной среды, рабочих поверхностей, рук персонала с определением бактериологических, вирусологических, микологических показателей с последующей макроскопической и микроскопической идентификацией микроорганизмов и идентификацией с применением автоматизированных систем времяпролётного матрично-ассоциированного лазерного масс-спектрометра на платформе MALDI-TOF, которая основывается на изучении масс-спектров рибосомальных белков в диапазоне 1000-10 000 дальтон и биоинформационного сравнения полученного спектра с базой данных референтных спектров и данных секвенирования микроорганизмов.</p></sec><sec><title>Результаты</title><p>Результаты. В результате проведённых исследований в помещениях многопрофильной клиники было выявлено микробное загрязнение условно патогенными грамположительными микроорганизмами, в том числе Staphylococcus aureus, и грамотрицательными бактериями, что представляет серьёзную эпидемиологическую опасность для пациентов этих палат независимо от профиля лечебного учреждения и требует обязательного контроля с учётом применяемых средств дезинфекции.</p></sec><sec><title>Заключение</title><p>Заключение. Собственные исследования и анализ отечественной и зарубежной литературы показали, что недостаточно осуществлять контроль воздушной среды в помещениях МОСТ только по показателю общего микробиологического загрязнения. Как в операционных, процедурных и перевязочных блоках, так и в палатах, физиотерапевтических, диагностических, лабораторных помещениях и вспомогательных помещениях также необходимо учитывать и другие санитарно-микробиологические показатели: общее микробное число, количество грамположительных палочек и кокков, в том числе Staphylococcus aureus, грибы, адено-, энтеро-, астровирусы, колифаги.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The work is devoted to evaluating the results of our own research of sanitary-microbiological monitoring of environmental objects in diversified treatment-and-prophylactic institutions of stationary type and of the analysis both of domestic and foreign data in order justify of the list of priority controlled sanitary-microbiological indices of air and surfaces to ensure the safety of hospital environment, medical organizations stationary type, regardless of their functional purpose.</p></sec><sec><title>Material and methods</title><p>Material and methods. The survey was conducted in various premises in the medical-prophylactic institutions of stationary type for two years. Studies included determination of microbial contamination of the air environment, working surfaces, hands of personnel with the detection of bacteriological, virological, and mycological parameters, followed by macroscopic and microscopic identification of microorganisms and identification using automated systems with the method of time-of-flight matrix-assisted laser mass spectrometry platform MALDI-TOF, based on the study of the mass spectra of ribosomal proteins in the range of 1000-10000 Daltons and bioinformatic comparison of the obtained spectrum with database reference spectra and PCR.</p></sec><sec><title>Results</title><p>Results. As a result, the research of surface washings in the premises of a multidisciplinary clinic revealed microbial contamination with conditionally pathogenic gram-positive cocci, including S.aureus, gram-positive and gram-negative bacteria posing a serious epidemiological danger to patients in these wards regardless of the MOST profile and requires mandatory monitoring taking into account of the used disinfectants.</p></sec><sec><title>Conclusion</title><p>Conclusion. Our own research and analysis of domestic and foreign literature showed that it is not enough to monitor the air in the MOST premises only in terms of total microbiological contamination.
