Химические вещества различных классов опасности относятся к числу наиболее распространённых вредных производственных факторов.</p><p>Цель исследования заключалась в изучении условий возникновения, а также структуры и распространённости профессиональных заболеваний от воздействия химических веществ на предприятиях в Арктике для исключения случаев досрочного ограничения или полного прекращения трудовой деятельности работников.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Изучены данные социально-гигиенического мониторинга «Условия труда и профессиональная заболеваемость населения Российской Федерации» в 2007–2020 гг.».</p></sec><sec><title>Результаты</title><p>Результаты. В Российской Арктике в 2007–2020 гг. доля химических веществ в структуре вредных производственных факторов составила 7,6%. Анализ многолетней динамики показывает тенденцию к её снижению. Вклад влияния химических веществ в структуру вредных производственных факторов составляет 7,8%. Для группы заболеваний, возникших вследствие воздействия данного фактора, характерно преимущественное поражение органов дыхания (74,6%) у работников никелевой промышленности (68,3%). Также отмечается формирование заболевания в более короткие сроки (24,3 ± 0,4 года), больший риск у женщин (относительный риск (ОР) = 3,48; доверительный интервал (ДИ) 2,91–4,16; p &lt; 0,001), больший риск злокачественных новообразований (ОР = 3,55; ДИ 3,13–4,02; р &lt; 0,001). Из 571 случая профессиональной патологии работников никелевого производства 355 (62,2%) были вызваны воздействием водонерастворимых соединений никеля, 188 (32,9%) случаев — гидроаэрозолей солей никеля и 28 (4,9%) — тетракарбонила никеля. Из 164 случаев интоксикаций 138 (84,1%) носили хронический характер и были обусловлены преимущественно соединениями никеля (87,7%). Основной причиной острых интоксикаций (n = 26) был оксид углерода (76,9%).</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Методы определения концентраций химических веществ в воздухе рабочей зоны не дают точной оценки экспозиции к ним работников, применяющих средства индивидуальной защиты.</p></sec><sec><title>Заключение</title><p>Заключение. Для уменьшения профессионального риска от воздействия химических веществ необходимо прежде всего снижение концентраций соединений никеля в воздухе рабочей зоны никелевых предприятий. В профилактике отдельных форм профессиональной патологии приоритет следует отдавать заболеваниям органов дыхания, формирующимся вследствие воздействия водонерастворимых соединений никеля, злокачественным новообразованиям и острым отравлениям оксидом углерода.</p><p>Соблюдение этических стандартов. Работа выполнялась в соответствии с принципами Хельсинкской декларации Всемирной медицинской ассоциации (в редакции 2013 г.). Протокол и дизайн исследования были обсуждены и одобрены локальным комитетом ФБУН «Северо-Западный научный центр гигиены и общественного здоровья» Роспотребнадзора 12.05.2021 г., протокол № 35.4. Chemicals of various hazard classes are among the most common harmful production factors.</p><p>We aimed to study conditions for the occurrence, prevalence and structure of occupational diseases from exposure to chemicals at enterprises in the Arctic in order to exclude cases of early restriction or complete termination of the labour activity of workers.</p><p>The study design was to analyze data of social and hygienic monitoring “Working conditions and occupational morbidity of the population” in the Russian Federation in 2007–2020.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We studied the data of social and hygienic monitoring “Working conditions and occupational morbidity of the population of the Russian Federation” in 2007–2020.</p></sec><sec><title>Results</title><p>Results. In the Russian Arctic in 2007-2020, the share of chemicals in the structure of harmful production factors was 7.6%. They were the cause of 7.8% of occupational diseases, the number of which tended to decrease. Characteristic for this group of diseases is the predominant damage to the respiratory organs (74.6%) in workers in the nickel industry (68.3%). Also, a feature of the diseases was the formation in a shorter time (24.3±0.4 years); greater risk in women (relative risk (RR) = 3.48, confidence interval (CI) 2.91-4.16; p&lt;0.001); greater risk of malignancy (RR=3.55; CI 3.13-4.02; p&lt;0.001). Out of five hundred seventy one case of occupational pathology of nickel production workers, 355 (62.2%) were caused by water-insoluble nickel compounds. Other 188 (32.9%) and 28 (4.9%) cases were due to hydroaerosols of nickel salts and nickel tetracarbonyl, respectively. Out of 164 cases of intoxication, 138 (84.1%) had a chronic course and were mainly caused by nickel compounds (87.7%). The main cause of acute intoxication (n=26) was carbon monoxide (76.9%).</p></sec><sec><title>Limitations</title><p>Limitations. Methods for determining concentrations of chemicals in the air of work areas do not provide an accurate exposure assessment in workers who use personal protective equipment.</p></sec><sec><title>Conclusion</title><p>Conclusion. To reduce the occupational health risks from chemicals, first of all, it is necessary, to decrease concentrations of nickel compounds in the air of the nickel enterprise working areas. In the prevention of certain forms of occupational pathology, priority should be given to respiratory diseases from exposure to water-insoluble nickel compounds, malignant neoplasms and acute carbon monoxide poisoning.</p><p>Compliance with ethical standards.
