Сохранение здоровья и трудового долголетия горняков, работающих на апатитовых рудниках, – одна из актуальных задач, сложность решения которой обусловлена высоким уровнем профессиональной заболеваемости этой категории работающих.</p><p>Цель исследования – анализ основных показателей профессионального здоровья горняков различных специальностей, работающих на открытых и подземных апатитовых рудниках, для минимизации риска развития профессиональной патологии.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Изучены следующие базы данных: результаты медицинского осмотра (921 горняк открытых рудников, 2649 горняков подземных рудников); реестр заболеваний Мурманского областного центра профессиональной патологии; реестр карт учёта профессионального заболевания (приказ Минздрава России от 28.05.2001 г. № 176) за 2008–2021 гг.</p></sec><sec><title>Результаты</title><p>Результаты. В 2008–2021 гг. профессиональная заболеваемость горняков открытых и подземных рудников, эксплуатирующих Хибинские апатитовые месторождения, была на уровне 208,62 и 170,95 случая на 10 000 работников соответственно. При подземной добыче в формировании профессиональных патологий большее значение имели повышенная тяжесть труда (60,7%) и локальная вибрация (10,1%), а у работников открытых рудников – общая вибрация (33,5%). В структуре патологии у горняков открытых рудников преобладала вибрационная болезнь (χ2 = 24,7; р &lt; 0,001), а у горняков подземных рудников – плечелопаточный периартроз (χ2 = 12,5; р &lt; 0,001), эпикондилит (χ2 = 7,44; р = 0,007), миофиброз предплечий (χ2 = 12,3; р &lt; 0,001), деформирующий остеоартроз (χ2 = 7,58; р = 0,006). Удельный вес работников с профессиональными болезнями был наибольшим среди проходчиков, бурильщиков подземных рудников и машинистов экскаватора: 40,8; 39,1 и 35,1% соответственно. Максимальная профессиональная заболеваемость регистрировалась у бурильщиков, а минимальная – у слесарей-ремонтников подземных рудников: 676,69 и 19,62 случая на 10 000 работников соответственно.</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Недостаточное для статистической обработки число наблюдений в некоторых группах работников.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты показывают, что при разработке сохраняющих здоровье мероприятий необходимо учитывать специальности горняков и особенности метода добычи апатитовой руды.</p><p>Соблюдение этических стандартов. Исследование одобрено локальным комитетом ФБУН «Северо-Западный научный центр гигиены и общественного здоровья» Роспотребнадзора (протокол № 35.5 от 12.05.2021 г.). Добровольного информированного согласия работников не требовалось, поскольку представленные материалы не содержат персональных данных.</p></sec><sec><title>Конфликт интересов</title><p>Конфликт интересов. The high level of occupational morbidity in apatite miners emphasizes the unresolved problem of maintaining their health throughout the entire period of their working activity.</p></sec><sec><title>Aim</title><p>Aim. To study the characteristics of the occupational health in miners of various occupations engaged in open-pit and underground apatite ore mining.</p></sec><sec><title>Material and methods</title><p>Material and methods. The following databases were examined: 1) the results of medical examination of nine hundred twenty one open-pit and 2649 underground apatite miners; 2) the register of occupational diseases of the Murmansk Regional Center for Occupational Pathology; 3) the register of occupational disease records (Order of the Ministry of Health of Russia dated May 28, 2001 No. 176) in 2008–2021.</p></sec><sec><title>Results</title><p>Results. In 2008–2021 occupational morbidity in underground and open-pit apatite ore miners at the Khibiny deposits reached 208.62 and 170.95 cases/10,000 workers. The increased labour severity (60.7%) and hand-arm vibration (10.1%) were of greater etiological importance in underground miners, while whole-body vibration played a more important role in open-pit miners (33.5%). In the structure of pathology, open-pit miners had a larger share of vibration disease (χ2=24.7; p&lt;0.001), and in underground miners – scapulo humeralis periarthrosis (χ2=12.5; p&lt;0.001), epicondylosis (χ2=7, 44; p=0.007), myofibrosis of the forearms (χ2=12.3; p&lt;0.001), deforming osteoarthritis (χ2=7.58; p=0.006). Occupational diseases developed most often in drifters, underground mine drillers, and open-pit excavator operators: 40.8%, 39.1% and 35.1% of workers. The maximum incidence rate was found among drillers, while the minimum rate was in underground repairmen: 738.92 and 19.62 cases/10,000 workers.</p></sec><sec><title>Limitations</title><p>Limitations. The number of observations in some groups of specialists is insufficient for statistical processing.</p></sec><sec><title>Conclusion</title><p>Conclusion. The new data obtained show that updated health-building measures should take into account the method of apatite ore mining and the specialty of the miners.</p></sec><sec><title>Keywords</title><p>Keywords: apatite ore mining; working conditions; miners’ health; occupational diseases; Kola Peninsula</p><p>Compliance with ethical standards.
