Проблема вибрационного воздействия остаётся актуальной в современной профпатологии. Современные эпидемиологические исследования показывают, что воздействие вибрации приводит к нарушению периферического кровообращения, спазму артериол и последующему развитию дистрофических изменений сосудистой стенки. В связи с этим цель нашего исследования – выявление особенностей динамики пульсационного индекса (PI) верхних конечностей при 1-й и 2-й стадиях вибрационной болезни (ВБ) от воздействия общей и локальной вибрации ультразвуковым методом.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Ультразвуковое сканирование артерий верхних конечностей проведено у 67 пациентов. На дистальном участке предплечья измеряли PI.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что при ВБ только от общей вибрации в отличие от сочетанного воздействия общей и локальной вибрации отмечается обратное соотношение общих темпов прогрессирования ангиодистонического синдрома, то есть более высокий темп роста наблюдается при 2-й стадии. Но при ВБ 1-й стадии от общей и локальной вибрации более выражены изменения по показателю PI, чем при ВБ 1-й стадии, от общей вибрации. На 2-й стадии ВБ при сохранении статистически значимых отличий разница в показателях заметно снижается.</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Основными недостатками ультразвукового сканирования являются продолжительность исследования и зависимость метода от оператора.</p></sec><sec><title>Заключение</title><p>Заключение. При более высокой степени ВБ независимо от вида вибрации (общая и локальная или только общая) уменьшается разница изменений по показателю PI, более выраженная на 1-й стадии болезни. Стандартизация протокола ультразвукового исследования верхних конечностей впоследствии поможет оценить динамику и уровень начальных ангиодистонических поражений, диагностика которых может предотвратить необратимые последствия хронической ишемии у пациентов на ранних этапах развития.</p><p>Соблюдение этических стандартов. Исследование проведено с соблюдением этических норм Хельсинкской декларации Всемирной медицинской ассоциации (ред. 2013 г.), одобрено Локальным этическим комитетом ФБУН «Северо-западный научный центр гигиены и общественного здоровья» Роспотребнадзора (протокол № 2022/50.2 от 28.12.2022 г.). The problem of vibration exposure remains relevant in modern occupational pathology. Modern epidemiological studies show exposure to vibration to be associated with peripheral circulatory disorders, arteriole spasm and the subsequent development of dystrophic changes in the vascular wall. </p></sec><sec><title>The purpose of the study</title><p>The purpose of the study. To identify the features of the trend in the pulsation index (PI) of the upper extremities in stages 1 and 2 of effects of vibration from exposure to whole-body and hand-arm vibration by ultrasound.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Ultrasound scanning of the arteries of the upper extremities was performed in sixty seven patients. In contrast to effects of vibration, the inverse ratio of the total growth rate has been established to be observed from whole-body and hand-arm vibrations only from the whole-body vibration, that is, a higher growth rate occurs at stage 2 of effects of vibration was measured on the distal part of the forearm.</p></sec><sec><title>Results</title><p>Results. However, at the same time, changes from whole-body and hand-arm vibration are more pronounced in effects of vibration stage 1.</p></sec><sec><title>Limitations</title><p>Limitations. The main disadvantages of ultrasound are the long-term and operator-dependent method.</p></sec><sec><title>Conclusion</title><p>Conclusion. With an increase in the degree of effects of vibration, regardless of the effects of such harmful production factors as whole-body and hand-arm and only whole-body vibration, the difference in changes in the PI index decreases, which is significantly pronounced at the 1st stages of the disease. Standardization of the protocol of ultrasound examination of the upper extremities will subsequently help to assess the dynamics and level of initial angiodystonic lesions in patients whose diagnosis can prevent irreversible consequences of chronic ischemia at the early stages of development.</p><p>Compliance with ethical standards. The study was conducted in compliance with the ethical standards of the Helsinki Declaration of the World Medical Association. The minutes of the meeting of the Local Ethics Committee of the Northwestern Scientific Center for Hygiene and Public Health No. 2022/50.2 dated 12/28/2022 were received.
