Повышение эффективности способа отбора проб с объектов внешней среды и пробоподготовки для первичного посева материала на микобактерии туберкулёза в ходе санитарно-бактериологического контроля является актуальным направлением совершенствования неспецифической профилактики туберкулёза.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследования (n = 2500) включали: приготовление суспензии M. tuberculosis H37Rv; подготовку и контаминацию тест-объектов (кафель, стекло, металл, пластик, лакокрасочное покрытие); подбор инструмента для отбора проб (мелкопористый поролон, ватный тампон, зонд-тампон универсальный), смывной жидкости (бульон по Ди-Ингли, 5%-й раствор Na3PO4, 0,85%-й раствор NaCl), деконтаминирующего раствора (10%-й раствор Na3PO4, NALC-NаOH); сравнительную оценку известного способа (№ 1) (приложение № 11 приказа Минздрава России № 109 от 21.03.2003 г.) и модифицированного способа (№ 2).</p></sec><sec><title>Результаты</title><p>Результаты. Эффективность способа № 2 за счёт использования зонд-тампона универсального, бульона по Ди-Ингли, NALC-NaOH значимо (p-value &lt; 0,001) превышает эффективность способа № 1 (мелкопористый поролон, 5%-й и 10%-й растворы Na3PO4) в 48 раз при концентрации Mtb в смывной жидкости 103 КОЕ/мл, в 22,3 раза – при концентрации 104 КОЕ/мл, в 8 раз – при 105 КОЕ/мл и в 4,6 раза – при 106 КОЕ/мл.</p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Разработанный способ позволяет получить рост КОЕ Mtb на среде Левенштейна – Йенсена при концентрации Mtb в смывной жидкости от 103 КОЕ/мл.</p></sec><sec><title>Заключение</title><p>Заключение. Показана высокая эффективность разработанного способа выделения Mtb с объектов внешней среды, превышающего в 48 раз по уровню высеваемости Mtb результаты, полученные стандартным способом в ходе санитарно-бактериологического контроля. Одновременно снижены трудозатраты, сокращено время на выполнение исследований. Стандартизирована методика в части исключения ручной подготовки инструмента, применения бульона по Ди-Ингли, положительно влияющего на жизнеспособность Mtb; сокращения времени деконтаминации с 18–24 ч до 40–45 мин. Полученные результаты позволяют рассматривать представленный способ как перспективный для включения в перечень методов санитарно-бактериологического контроля M. tuberculosis.</p><p>Соблюдение этических стандартов. Improving the efficiency of the method of washouts from environmental objects and sample preparation for the primary seeding of material for Mycobacterium tuberculosis (Mtb) during sanitary and bacteriological control is an urgent area of improvement of nonspecific tuberculosis prevention.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. The studies (n=2500) included: suspension preparation Mtb H37Rv); preparation and contamination of test-objects (tile, glass, metal, plastic, painted surface); selection of tools for washouts (fine-pore foam, cotton swab, universal tampon probe), solution for washout (Dey-Engley broth, 5.0% Na3PO4, 0.9% NaCl), solution for decontamination (10.0% Na3PO4, NALC-NaOH); comparative of the efficacy of two methods for the isolation of Mtb (known method No.1 and modified method No.2). </p></sec><sec><title>Results</title><p>Results. The efficiency of the method No. 2 (due to the use of: universal tampon probe; Dey-Engley broth; NALC-NaOH) significantly (p-value &lt;0.001) exceeds the efficiency of method No. 1 (fine-pore foam, 5.0% and 10.0% Na3PO4 solutions) by 48.0 times at the concentration of Mtb in the solution of washout at 103 CFU/ml, by 22.3 times – at the concentration of 104 CFU/ml, by 8.0 times – at 105 CFU/ml and by 4.6 times – at 106 CFU/ml.</p></sec><sec><title>Limitation</title><p>Limitation. The modified method allows obtaining growth of CFU of Mtb on Levenshtein-Jensen at the concentration of Mtb in the solution of washout at 103 CFU/ml.</p></sec><sec><title>Conclusions</title><p>Conclusions. High efficiency of the developed method of Mtb isolation from the objects of external environment (exceeding by 48 times the results obtained by the standard method) during sanitary and bacteriological control, reduction both of labor costs and the of time for research, standardization of the method in terms of exclusion of tools prepared by “manual” method; use of Dey-Engley broth, positively affecting the viability of Mtb; reduction of decontamination time from 18–24 hours to 40–45 min – allow considering the presented method as a promising one for inclusion in the list of methods of sanitary and bacteriological control in relation to Mtb.</p><p>Compliance with ethical standards.
