Cronobacter sakazakii – облигатный патоген для детей раннего возраста с ослабленным иммунитетом, вызывающий некротизирующий энтероколит, менингит и сепсис. Основным фактором передачи данных инфекций являются сухие смеси для искусственного вскармливания. Наряду с C. sakazakii в сухих продуктах для детского питания выявляются и другие энтеробактерии, некоторые из которых относятся к оппортунистическим патогенам. </p><p>Поскольку все эти бактерии могут проявлять терморезистентность, переживая такой фактор технологического стресса, как сушка при высоких температурах, возможно наличие у них других изменённых свойств, к которым относится и антимикробная резистентность (АМР). </p><p>Цель исследования – оценка профиля АМР, в том числе к клинически значимым антимикробным препаратам (АМП), у бактерий C. sakazakii и сопутствующих им энтеробактерий, изолированных из циркулирующих на рынке Российской Федерации сухих продуктов для детей раннего возраста. </p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследовали 177 образцов сухих молочных смесей и компонентов для их производства методом бакпосева с двухстадийным обогащением с последующей биохимической идентификацией изолятов. У выделенных грамотрицательных бактерий и сопутствующих изолятов изучен фенотипический профиль АМР диско-диффузионным методом, для изолятов C. sakazakii, Klebsiella spp., Acinetobacter baumannii дополнительно устанавливали значения минимальных ингибирующих концентраций (МИК). </p><p>В экстрактах ДНК у выделенных штаммов исследовали гены, кодирующие бета-лактамазы расширенного спектра (tem, shv, ces, kpc, ctx), карбапенемазы (ges, kpc, oxa) и металло-бета-лактамазы (imp, vim), методом ПЦР с использованием набора БакРезиста GLA. Исследование гена семейства RND, кодирующего эффлюкс АМП из клетки и множественную резистентность adeF, производили с помощью ПЦР в реальном времени с предварительно подобранными специфичными праймерами, с последующим подтверждением в гель-электрофорезе.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено, что 8,5% сухих продуктов для детского питания были контаминированы бактериями Cronobacter spp., а 38,4% – другими терморезистентными Enterobacteriaceae. Всего изучено 65 штаммов: Cronobacter spp. (15 штаммов) и сопутствующие Enterobacteriaceae spp. (49), Acinetobacter baumannii (1). Фенотипически все штаммы Cronobacter spp., а также Klebsiella spp., A. baumannii проявляли высокий уровень АМР к антибиотикам бета-лактамного ряда, в том числе приоритетного класса (карбапенемам), и обладали множественной АМР к 2–13 антибиотикам. Изучение генного потенциала АМР к бета-лактамным антибиотикам у Cronobacter и других терморезистентных бактерий из сухих детских продуктов показало наличие генов бета-лактамаз расширенного спектра действия (БЛРС) у оппортунистических патогенов Klebsiella pneumoniae, Acinetobacter baumannii, относимых к группе ESKAPE. </p></sec><sec><title>Ограничения исследования</title><p>Ограничения исследования. Не исследованы механизмы генетического потенциала резистентности, подтверждающие трансмиссивную природу АМР.</p></sec><sec><title>Заключение</title><p>Заключение. При контроле Cronobacter spp. в сухих продуктах для грудных детей целесообразно идентифицировать сопутствующие терморезистентные грамотрицательные бактерии. При обнаружении среди них представителей группы ESKAPE важно изучать наличие у них АМР к клинически значимым АМП и факторов патогенности. Полученные данные будут полезны для будущих рекомендаций по контролю качества компонентов и эффективности режимов производства сухих молочных смесей и каш в отношении терморезистентных бесспоровых бактерий для снижения рисков попадания болезнетворных патогенов.</p><p>Соблюдение этических стандартов. Cronobacter sakazakii (C. sakazakii) is an obligate pathogen in neonates, causing necrotizing enterocolitis, meningitis, and sepsis. The main transmission factor for these infections is powdered infant formula. Along with C. sakazakii, other Enterobacteriaceae spp., some of which are opportunistic pathogens, have been detected in dry foods for infants. Since all these bacteria are capable of exhibiting thermoresistance, surviving a technological stress such as the high-temperature drying process, there is a possibility that they also have other altered properties, in particular, antimicrobial resistance (AMR). </p></sec><sec><title>Objective</title><p>Objective. To evaluate the AMR profile, including resistance to clinically significant antimicrobial drugs, in C. sakazakii and other associated Enterobacteriaceae spp. isolated from dry foods for infants available on the Russian market.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. A total of one hundred seventy seven samples of dry foods for infants and their production components were analyzed using bacterial culture with two-stage enrichment followed by biochemical identification of the isolates. The phenotypic AMR profile of the isolated Gram-negative and associated isolates was studied using the disk-diffusion method. For C. sakazakii, Klebsiella spp., and Acinetobacter baumannii isolates, minimum inhibitory concentrations (MICs) were additionally determined. DNA extracts from the isolated strains were analyzed by PCR for genes encoding extended-spectrum β-lactamases (tem, shv, ges, kpc, ctx), carbapenemases (ges, kpc, oxa), and metallo-β-lactamases (imp, vim) and the multidrug efflux pump gene adeF. </p></sec><sec><title>Results</title><p>Results. Among dry foods for infants, 8.5% were contaminated with Cronobacter spp. and 38.4% - with other thermoresistant Enterobacteriaceae. A total of 65 strains were studied – 15 Cronobacter spp. and 49 associated Enterobacteriaceae spp. Phenotypically, all Cronobacter spp., as well as Klebsiella spp. and A. baumannii strains exhibited high levels of AMR to β-lactam antibiotics, including carbapenems as a priority class, and possessed multidrug resistance to 2–13 antibiotics.Analysis of the genetic potential of AMR to β-lactam antibiotics in Cronobacter and other thermoresistant bacteria from dry foods for infants revealed the presence of extended-spectrum β-lactamase (ESBL) genes in the opportunistic pathogens Klebsiella pneumoniae and Acinetobacter baumannii, which are classified as the ESKAPE group.</p></sec><sec><title>Limitations</title><p>Limitations. The mechanisms underlying gene potential for resistance that would support the transmissible nature of AMR, have not been investigated.</p></sec><sec><title>Conclusion</title><p>Conclusion. When monitoring Cronobacter spp. in dry foods for infants, it is advisable to identify accompanying thermoresistant Gram-negative bacteria. If representatives of the ESKAPE group are detected among them, it is important to study their AMR to clinically significant antimicrobial drugs and their virulence factors. The data obtained will be useful for future recommendations on the quality control of ingredients and the effectiveness of production modes for dry foods for infants and cereals against thermoresistant non-spore-forming bacteria to reduce the risk for contamination with pathogens.</p><p>Compliance with ethical standards.
